The Competency Based Fieldwork Evaluation for Occupational Therapists (CBFE-OT)

(Bossers, A., Miller, L.T., Polatajko, H.J., Hartley, M. 2007) is the primary method of evaluation of student performance in clinical practice. Each student will bring her/his copy of the CBFE-OT manual to placement for the evaluation process. Due to copyright regulations the CBFE-OT cannot be posted.

Student Evaluation of Practicum and Preceptor

This form should be completed by the student at the end of the practicum, discussed with the preceptor/ site student coordinator and returned to the school with the final evaluation forms within one week following the end of the practicum.

If you are already a preceptor with the MSc OT Program we kindly ask that you complete and return this form any time your information changes for inclusion in our database.   This database is for use by the School of Rehabilitation staff and Faculty only.  It will not be shared with students or other parties unless you have indicated otherwise on the form.
